HIPAA Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our Commitment
SCIER Labs Diagnostics is required by federal law to maintain the privacy of your Protected Health Information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
How We May Use and Disclose Your PHI
We may use or disclose your PHI for the following purposes:
- Treatment — to provide, coordinate, or manage your diagnostic imaging services and related care. For example, sharing your echocardiogram results with the ordering physician.
- Payment — to bill and collect payment for services provided. This may include sharing information with your insurance company or health plan.
- Healthcare Operations — to support our business activities, including quality assessment, credentialing, auditing, and compliance programs.
- As Required by Law — when federal, state, or local law mandates disclosure, such as public health reporting or responding to court orders.
Your Rights
Under HIPAA, you have the right to:
- Access — request a copy of your PHI maintained by us.
- Amendment — request corrections to your PHI if you believe it is inaccurate or incomplete.
- Accounting of Disclosures — request a list of certain disclosures we have made of your PHI.
- Restriction Requests — request restrictions on certain uses or disclosures, though we are not required to agree to all requests.
- Confidential Communications — request that we communicate with you by alternative means or at alternative locations.
- Breach Notification — receive notification if there is a breach of your unsecured PHI.
Our Security Practices
- Business Associate Agreements (BAAs) are in place with all vendors that handle PHI on our behalf.
- PHI is transmitted using encryption that exceeds HIPAA Security Rule standards.
- Access to PHI is limited to authorized personnel on a need-to-know basis.
- Regular risk assessments and workforce training are conducted to maintain compliance.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
